You will need to download or print three sets of forms. They are posted below in .pdf format. If you need Adobe PDF Reader, you can download that from the Adobe website as well.

The ministerial profile is what we use instead of a resume. The disclosure form is a standard form for all Disciples. Please fill out both the profile and the disclosure form and send to me. I will add them to your file or start a file for you.

The reference form needs to be given to four people to fill out on your behalf. If you are transferring from another denomination, please use a denominational official where it asks for “regional staff person”. If you have never had a license nor been ordained, please use your pastor for “regional staff person”. The other three should be people who can share their evaluations of you with our Ministry Committee.

Remember: All forms are to be returned to me, not to the Search and Call office in Homeland Ministries in Indianapolis.
